After completing this application, you will be sent an email with orientation materials and a link to our quiz. This quiz will determine if we decide to proceed with your application. If we move forward, you will receive a response within 10 business days that will allow you to register for onsite training.

Intent, Acknowledgements and Releases
By signing below, I hereby request consideration and assignment to one of the volunteer position, as stated below.
These forms must be returned to the HHS Program Manager before I begin working as a volunteer. I understand and
agree that the following terms, conditions and understandings with Harbor Humane Society (HHS) apply.

Terms and Conditions
1. My services to HHS are provided strictly in a voluntary capacity. As a volunteer, I provide my services without any
expressed or implied promise of salary, compensation, or other payment of any kind, whatsoever.
2. My services are furnished without any employment type benefits, including insurance programs, worker
compensation, accrual in any form of annuity, vacations or sick time.
3. I will familiarize myself with and comply with HHS’s policies and procedures applicable to both the staff and
volunteers. I fully understand that HHS expects high standards of moral and ethical treatment of the animals under
its care. I will adhere strictly to these standards in my capacity as a Volunteer.
4. I understand that HHS, without notice of hearing, may limit or terminate my services as a volunteer at any time, with
or without reason.

Tetanus Waiver
Due to the nature of volunteer activities, Harbor Humane Society feels it is important for all volunteers to have an up to
date tetanus vaccination.

I understand that because I may handle animals, equipment and materials during my volunteer activities, it is important
to discuss being vaccinated with my physician. I release HHS from all responsibility for the consequences of not seeking
medical advice and/or obtaining a tetanus vaccination.
I have read and agree to the above terms, conditions and tetanus waiver.

Waiver and Hold Harmless
I acknowledge that I will be performing volunteer services for Harbor Humane Society, a Michigan Non-Profit Corporation
(the HHS). I further acknowledge that there I will encounter risk, such as, but not limited to, being bitten or scratched by
animals, being exposed to zoonotic diseases, and accidents involving use of equipment or while engaging in activities
such as cleaning and maintenance.

I understand that the handling of animals and other Volunteer activities on behalf of the HHS may place me in hazardous
situations and could result in injury to me and damage to my personal property. On behalf of myself, and my heirs,
personal representatives and assigns; I hereby release discharge, a indemnify and hold harmless HHS and its directors,
officers, employees, volunteers and agents from any and all claims, causes of action and demands of any nature,
whether known or unknown, arising out of or in connection with my Volunteer activities on behalf of HHS.

In consideration of being permitted to perform such volunteer serviced for HHS, the undersigned voluntarily and
knowingly executes this waiver with the express intention of waiving all rights, claims or causes of action involving,
without limitation, bodily injury of property damage to the undersigned, to his/her family members and to their domestic
pets or other animals resulting from the undersigned’s performance of the volunteer services contemplated herein.
I agree to hold harmless Harbor Humane Society, its officers, directors, agents, employees and volunteers from and
against any and all liability, damage loss, cost and expense incurred as a result of any claim, demand or cause of action,
brought against HHS its officers, agents, employees, or volunteers jointly or individually, for bodily injury or property
damage suffered as a result of the undersigned’s negligent, reckless or willful actions in the performance of the
volunteers services contemplated herein or as a result of the failure to perform the volunteer services contemplated
herein.

I further acknowledge I have been advised to secure any necessary insurance needed to serve as a volunteer for Harbor
Humane Society. I have read, understand and agree to the above.

Photographic Release
Understanding that public relations are an important part of a Volunteer’s activities on behalf of HHS, I hereby authorize
the HHS to use any photographs of me in its possession for public relations purposes. I ask that HHS use reasonable
efforts to give me advance notice of any such used, but such notification is not a condition to release of photographs for
public relations purposes.
